Heath Kohlmeier MSN, FNP-C


Erin Nester MSN, PMHNP-C

Heath, a Family Nurse Practitioner in Family Practice, joins our medical team of
Anne Butsch MD and Melissa Hall DNP, FNP-BC.  He will be in the office
Monday-Friday as a full time provider.  Heath is a native of Gibson County and is
pleased to serve his community and neighbors.

Erin is a Psychiatric Mental Health Nurse Practitioner.  She will be at Tulip Tree
in a full time position joining Ethel Elkins LCSW in our behavioral health program. 
Erin’s training allows her to treat patients who need psychiatric level of care for
mental health or addiction treatment.  


Marketplace Open Enrollment ended DECEMBER 15th.  Only individuals with a qualifying event may sign up now.  The Marketplace plans for 2019 are not affected by current court actions and appeals.

GOOD NEWS:  Eligible Indiana Residents may sign up for HIP 2.0             coverage all year long.

Federally Qualified Health Center Look Alike (FQHC LAL) as of July 1, 2018. 

We are a Federally Qualified Health Center Look-Alike (FQHC-LAL) located in Fort Branch in Gibson County Indiana.  Our mission is to serve people who are uninsured and underinsured by providing high quality, affordable, compassionate and accessible medical, dental, and behavioral health care.

Our patients come to us from throughout southwest Indiana and neighboring counties of Illinois.  We accept all Medicaid, HIP 2.0, Medicare, Affordable Care Act insurance coverages and most commercial plans.  We are always accepting new patients whether you have insurance or not.

Tulip Tree Family Health Care opened its doors in 1999 and we have been growing to meet the needs of our patients ever since.  Our clinic is supported by fees paid by patients and insurance companies for services, grants from the Indiana State Department of Health, grants from private foundations and businesses, and individual donations.
