Who is eligible for the Sliding Fee Discount Program?

Sliding Fees are offered to everyone with household incomes below 200% of the Federal Poverty Level.  A Nominal Visit Fee is offered to patients whose household income is below 100% of the Federal Poverty Level.

The best and most accurate way to determine what your individual sliding scale fees would be is to call the office and schedule an Intake Appointment.  At an Intake Appointment, you will meet with an Intake Coordinator to calculate your discount and take care of new patient paperwork.

I want to sign up for a Sliding Fee Discount Plan, what do I need to bring to my Intake Appointment?

You will need to bring basic income information for each person living in your household.  You will also need a picture ID for each adult who wants to be a patient.

For everyone that lives in your household, please bring the following:

  • Last year’s Federal Tax return, including all attachments (not just the W-2)
  • Last 2-3 paycheck stubs
  • Unemployment benefits statement
  • Statement of Social Security benefits
  • Statement of all pension benefits
  • Current Food Stamps benefit letter or statement from the Food Stamp office
  • Statement from the Clerk’s office of child support received or paid
  • Statement of any other form of income coming into your household
  • If you are a full time student, bring a copy of your current schedule
  • Picture ID of each adult wanting to become a patient

If you have any questions, please call or stop by the office before your intake appointment so we may assist in making sure you have all the appropriate information required.

If there are items on the list that you do not have, it is okay – still come to the Intake Appointment and talk about your individual situation with your Intake Coordinator.

How soon can I be seen for treatment? Will I be seen by the Dentist/Nurse Practitioner/Physicial the same day as my Intake Appointment?

As soon as your paperwork has been completed and the discount has been calculated, you will be able to be seen at the first available appointment – it may even be the same day.

I don't really feel comfortable providing my income information, can I still get a discount?

If you do provide your household income information, it becomes part of your confidential record and we are extremely respectful of that information.  If you still decide you are not comfortable, you may benefit from our slightly reduced self pay rates for patients who pay for their care before or on the day of their visit.  You can call the office for more information on this option.

I don't have any income at all, so I won't have to pay anything, right?

Unfortunately, we are not a free medical or dental clinic.  However, most people who complete their Intake Appointment are eligible for a significant discount.  We also never turn away a patient who has an emergent medical need.  We offer VERY affordable payment arrangements tailored to your individual situation.  Patients with household incomes below 100% of the Federal Poverty Level are given special consideration.  The best, and really the only way, to fully understand our program is to complete an Intake Appointment.  Call the office today to schedule yours!
